Do Support Workers Need to Keep Notes? What the NDIS Actually Expects

Do support workers really have to write notes?

Yes. If you are providing support under the NDIS, documenting what you do is not optional. Whether you are an independent support worker, a contractor through a provider, or working under a self-managed or plan-managed participant, you are expected to keep accurate records of each shift.

What does the NDIS expect from notes?

The NDIS does not give you a fixed template, but it expects evidence of what support was provided, that it matches the participant's goals and plan, that you are working within your scope, and that incidents, issues, or concerns are noted and traceable.

If your notes are ever audited, and yes, it happens, they need to show:

  • Who you supported
  • What you did
  • When you did it
  • Why it was reasonable and necessary

No note means no proof. And that can mean returning funds, or worse.

"But I've never been asked for notes before..."

That is common, until it isn't. Audits can be random, participants may request documentation, and plan managers may hold payment if a service is questioned.

Notes protect you from complaints or liability, protect the participant if something needs to be clarified later, and protect everyone when support needs are reviewed or escalated.

What should a good shift note include?

  • Client initials or de-identified name
  • Date and time of the shift
  • What support you provided
  • Any progress toward goals
  • Issues, changes, or concerns observed
  • Signed off by you
