A support worker invoice needs your details and ABN, who it is going to, an invoice number and date, the dates you worked, the NDIS support item code, your hours and rate, and the total. Most NDIS supports are GST-free. The template below is free to copy or download.

What every NDIS invoice needs

Plan managers pay faster when the invoice has everything they need to claim:

  • Your details and ABN. Your name or business name, your ABN, and your payment details.
  • Who it is going to. The participant, or their plan manager, with the details they gave you.
  • Invoice number and date. A unique number and the date you issued it.
  • The support item code. The NDIS support item code and description, for example 01_011_0107_1_1. This tells the plan manager what to claim.
  • Dates, hours and rate. When you worked, how many hours, and your rate for the day and time.
  • GST. Most NDIS supports delivered to a participant are GST-free, so many sole trader support workers do not add GST. Whether GST applies can depend on your registration and the support, so confirm your situation with your accountant or the ATO.
